Housing Outreach Specialist

Responsible for partnering with Case Managers to locate and engage homeless members who may be eligible for the Florida Housing Pilot Program, as well as developing and implementing the health plans housing strategy and programs. This includes the approach to assisting individuals in programs in securing accessible, affordable housing through Federal and local programs. Primary duties include, but are not limited to:

* Field-based outreach in assigned region to locate and engage members. Region 5: Pasco, Pinellas; Region 7: Brevard, Orange, Osceola, Seminole. Due to high concentration of field work, candidate must reside in one of counties in regions 5 or 7.
* Liaise between housing agencies and the health plan and partner with these agencies to develop and access affordable housing services for members.
* Support Case Managers in identifying housing options for members.
* Associate's degree required. Bachelor degree preferred.
* Requires 1 year of experience in developing housing and/or housing policy at a local, state or federal level for special populations such as the elderly, people with disability and/or people with mental health challenges to secure accessible, affordable housing through Federal and local programs
* Ability to leverage knowledge and experience of affordable housing programs for identified populations at a local, state and federal level to identify and develop housing opportunities
* Or any combination of education and experience, which would provide an equivalent background.
* Travel required.
